NOTICE TO PATIENTS AND COMMUNITY:

Our number one concern is the safety of our patients, staff, and community. You will notice additional precautions we have put in place to protect you and our community.

Prior to entry into our facilities all patients and guests will be screened with the following criteria:

1. Do you have a fever, cough or shortness of breath, AND
2. Have you been in close contact with a person known to have COVID-19
-or-
do you live in or have recently been in an area with the ongoing spread of COVID-19.

For your protection, and the protection of our community, we ask that you answer these questions truthfully and to the best of your ability. Rest assured we will take great care of you either way.

If you meet any of the criteria above you will be asked to return to your car so that you can be assessed, and if necessary tested for COVID-19 by one of our providers.
